How is the registration form information used?
Advertising offers with registration forms require users to provide contact information (e.g., name, e-mail address, phone number, and physical address) and demographic information (e.g., gender, age, and insurance carrier). We use the information from the registration form to contact users according to their requests, and to further fulfill requests, as well as to send our users information about our website. This contact information will only be provided to the specific sponsors of offers, or providers of services that you have expressly selected. User specifically grants Alliance Health and selected sponsors or service providers permission to contact the user by telephone and/or email, including phone calls that use pre-recorded messages.
